Low levels of physical activity in individuals with chronic pain can lead to additional functional impairment and disability. This study aims to investigate the predictors of low physical activity levels in individuals with chronic pain, and to determine the accuracy of the artificial neural network used to analyze these predictors. Community-dwelling older adults with chronic pain (n = 103) were surveyed for their physical activity levels and classified into low, moderate, or high physical activity level groups. Chronic pain-related measurements, physical function assessment, and clinical history, which all influence physical activity, were also taken at the same time. Logistic regression analysis and analysis of multilayer perceptron, an artificial neural network algorithm, were performed. Both analyses revealed that history of falls was a predictor of low levels of physical activity in community-dwelling older adults. Multilayer perceptron analysis was shown to have excellent accuracy. Our results emphasize the importance of fall prevention in improving the physical activity levels of community-dwelling older adults with chronic pain. Future cross-sectional studies should compare multiple analysis methods to show results with improved accuracy.

Robotic Hand glove is one of the most commonly used technique in the rehabilitation systems. In this paper, we developed a robotic hand system with a proposed sensing mechanism-based AI algorithm, which can acquire grasping forces from human fingers. It is composed of five low-cost force sensors attached to the glove's fingertips and wireless data logger. Several experiments including grasping a plastic bottle and squeezing a tennis ball are implemented to verify the efficiency of the proposed system using the developed glove. As a result, it accurately estimates the forces applied by each finger with the aim of achieving a secure grasp comparison with conventional methods.

軟性膝装具の装着圧力の変化が，変形性膝関節症患者の立ち上がり動作に与える影響を調べた．対象は，変形性膝関節症患者7名とした．異なる装着圧力の軟性膝装具を使用し，立ち上がり動作離殿時の膝関節屈曲角度を分析した．立ち上がり動作離殿時の膝関節屈曲角度は，Control条件と比較してHigh Pressure条件にて有意に大きくなり，装着圧力を高くすることにより増加傾向を示した．軟性膝装具の装着圧力を高くすることにより，立ち上がり動作離殿時の膝関節屈曲角度が大きくなり，効率的な立ち上がり動作が遂行できることが明らかとなった．

This paper describes a method to eliminate multipath effects by using the shield cover of a microwave absorber to improve Global Positioning System (GPS) positioning accuracy. We studied a walking movement support system for visually impaired people. The system provides audio guidance regarding the direction of movement and pedestrian signals based on the location of the intersection and the pedestrian’s GPS position. Because a pedestrian device is a wearable terminal, we want to obtain highly accurate location information with a simple mechanism that can be comfortably worn on the body. In this study, we propose a method using a shielding cover with a microwave absorber to improve the GPS positioning accuracy in environments where multipath, a major cause of positioning error, occurs. This method does not require any modification to the hardware or software of the GPS receiver, and the multipath is prevented by adjusting only the height and position of the shielding cover. In a walking experiment using the proposed method, we confirmed that the shielding plate, set at an open angle of 90 degrees, can provide positioning without being affected by multipath in an environment where there are buildings on both sides of the path.

Hallux valgus, a frequently seen foot deformity, requires early detection to prevent it from becoming more severe. It is a medical economic problem, so a means of quickly distinguishing it would be helpful. We designed and investigated the accuracy of an early version of a tool for screening hallux valgus using machine learning. The tool would ascertain whether patients had hallux valgus by analyzing pictures of their feet. In this study, 507 images of feet were used for machine learning. Image preprocessing was conducted using the comparatively simple pattern A (rescaling, angle adjustment, and trimming) and slightly more complicated pattern B (same, plus vertical flip, binary formatting, and edge emphasis). This study used the VGG16 convolutional neural network. Pattern B machine learning was more accurate than pattern A. In our early model, Pattern A achieved 0.62 for accuracy, 0.56 for precision, 0.94 for recall, and 0.71 for F1 score. As for Pattern B, the scores were 0.79, 0.77, 0.96, and 0.86, respectively. Machine learning was sufficiently accurate to distinguish foot images between feet with hallux valgus and normal feet. With further refinement, this tool could be used for the easy screening of hallux valgus.

Improved YOLOv5-Based Lightweight Object Detection Algorithm for People with Visual Impairment to Detect Buses 査読有り国際誌

Object detection is crucial for individuals with visual impairment, especially when waiting for a bus. In this study, we propose a lightweight and highly accurate bus detection model based on an improved version of the YOLOv5 model. We propose integrating the GhostConv and C3Ghost Modules into the YOLOv5 network to reduce the number of parameters and floating-point operations per second (FLOPs), ensuring detection accuracy while reducing the model parameters. Following that, we added the SimSPPF module to replace the SPPF in the YOLOv5 backbone for increased computational efficiency and accurate object detection capabilities. Finally, we developed a Slim scale detection model by modifying the original YOLOv5 structure in order to make the model more efficient and faster, which is critical for real-time object detection applications. According to the experimental results, the Improved-YOLOv5 outperforms the original YOLOv5 in terms of the precision, recall, and mAP@0.5. Further analysis of the model complexity reveals that the Improved-YOLOv5 is more efficient due to fewer FLOPS, with fewer parameters, less memory usage, and faster inference time capabilities. The proposed model is smaller and more feasible to implement in resource-constrained mobile devices and a promising option for bus detection systems.

Many caregivers experienced lower back injuries by patient transfer motion. Patient transfer motion causes lumbar load since this task requires lifting and twisting. For this problem, previous studies explored the suitable posture such as trunk angle to reduce lumbar load in patient transfer motion. However, it is difficult to adjust these postures for caregivers because these postures should be dynamically adjusted in patient transfer motion. Thus, we focus on initial foot placement to reduce lumbar load, because initial foot placement is adjustable before starting transfer. Therefore, the objective of this study is to find suitable initial foot placement for patient transfer motion. In the experiment, 10 healthy males performed simulated patient transfer motion with anteroposterior (AP) and mediolateral (ML) foot placement. Erector spinae muscle activity was measured during patient transfer by the surface electromyography (sEMG). Furthermore, compressive force of L4-L5 was calculated by the computational musculoskeletal simulator. The results showed that AP foot placement provided smaller compressive force of L4-L5 than ML. On the other hand, there was no significant difference for erector spinae muscle activity between AP and ML foot placement. These findings will contribute to determining suitable initial posture to prevent lower back injuries in patient transfer motion.

Nurses and caregivers experienced lower back pain (LBP) as a result of patient handling motions to assist in wheelchair or bed transfer and repositioning. Therefore, previous studies provided assistive tools and devices to reduce lumbar loads while these patients handling motions. However, these devices and tools could not be used in several facilities because these facilities require comfortability, time efficiency, and low cost. Therefore, from these backgrounds, LBP prevention is required for any facility and situation without assistive devices and tools. Low load posture and movement of patient handling motions should be provided for LBP prevention without devices and tools. In our previous study, we explored suitable foot placement to reduce the lumbar load during turning a patient on a bed because patient repositioning on a bed is a serious cause of LBP. However, our previous study indicates that lumbar loads while turning patients cannot be improved only by foot placement adjustment. Postural asymmetry is related to lumbar load and cannot be avoided with only suitable foot placement. On the other hand, adjustment of both arm movement and foot placement has a possibility that reduces lumbar loads due to asymmetry posture. This study aims to explore a suitable combination of foot placement and arm movement order while turning patients for LBP prevention. A total of nine lumbar load combinations consisting of three foot placements (parallel stance, left forward, and right forward) and three arm movement orders (parallel, right first, and left first) were investigated via surface electromyography (sEMG) measurement during patient turning. The results of sEMG showed that a combination using anteroposterior foot placement (right forward) and parallel arm movement (both left and right arm operated at the same time) performed turning patient with the smallest activity for both left and right erector spinae muscles. These results indicate that a combination of anteroposterior foot placement and parallel arm movement could provide a suitable posture to reduce lumbar loads during patient turning.

[Purpose] To identify the lumbar loading movements necessary in clinical practice. [Participants and Methods] A questionnaire survey was conducted among physical and occupational therapists in Japan. There were no exclusion criteria regarding the number of years of experience, age, or field of employment. The participants were randomly selected and administered the questionnaire. They were asked to list and rank the lumbar loadings they considered necessary. [Results] A total of 739 respondents participated in the survey. The results of this nationwide survey indicated that the lifting movement of heavy objects in the trunk flexion position was the most common movement (for 354 participants). [Conclusion] The main loading movements of the lumbar spine were reported to be heavy lifting movements (in the trunk flexion position) and trunk rotation movements. As perspectives, we aim to conduct an analytical study of some of lumbar spine loading movements outlined in this study, using a musculoskeletal simulator and electromyography.

Supporting the elderly by utilizing their remaining functions will lead to the extension of their healthy life span. In this study, we developed a portable cane-type standing and sitting positional mobility support device. In this study, we developed a cane-type mobility support device, and selected a suitable support direction for elderly people by dynamic analysis simulation. In addition, the necessary assisting force for the elderly was examined and the assisting force was set. The effectiveness of the device was evaluated by measuring the surface electromyogram of healthy subjects. As a result, it was found that the specifications of this device were effective when the assist direction was 30 degrees and the assist force was 110 N. From the measurement of the surface electromyogram, it was found that the device was effective for both standing and sitting motions. It also helps to ensure the safety of the sitting motion of elderly people.

変形性膝関節症患者は、膝装具装着により関節位置覚が改善することが報告されているが、面ファスナーの締め具合を示す装着圧力には着目されていないことが多い。装着により局所圧迫の神経生理学的な効果が得られているため、装着圧力を高めることにより、さらに関節位置覚が改善する可能性がある。そこで、本研究は変形性膝関節症患者を対象に膝装具の装着圧力に着目し、関節位置覚に与える影響を検証した。結果より、膝関節屈曲60°においては、High PressureがControlと比較して有意に低い値を示した（p ＜ 0.05）。軟性膝装具の装着圧力を高くすることにより、関節位置覚が向上する可能性が示唆された。

[Purpose] This study aimed to develop and validate a method for identifying factors that may cause a fall during the pre-impact fall period using wearable sensors. [Participants and Methods] The participants were 23 young people from the public data set (mean age, 23.4 years). Acceleration and angular velocity information obtained from sensors attached to the participant’s waist was used to generate the pre-impact fall. The cause of the fall (slip, trip, fainting, get up, sit down) was then classified with and without the addition of activity of daily living data using three different support vector machine. In addition, we investigated the influence of lead time (0–2.0s) on accuracy. [Results] The quadratic and cubic support vector machine identified the activity of daily living and fall patterns more accurately than the linear support vector machine, and the cubic support vector machine was better for classification, although the difference was slight. The greatest accuracy for predicting the cause of the fall (87.9%) was obtained when the cubic support vector machine was used, activity of daily living was factored into the analysis, and the lead time was 0.25 sec. [Conclusion] Support vector machine can identify the cause of the fall during the pre-impact fall period. Appropriate individualized interventions may be designed based on the most likely cause of fall as identified by this analysis method.

Unsuitable foot clearance and step length lead falling accidents; thus, these gait parameters should be monitored for fall prevention. According to previous study, users prefer wrist as a position to wear wearable device for daily gait monitoring. Therefore, we have been developing gait monitoring method using wearable device on a wrist. Our previous method classifies gaits with different foot clearance or step length by machine learning technique using 3-axis acceleration data obtained from wearable device on a wrist. The results of our previous study showed that this method could classify gaits for each step. However, accuracy of our previous method was insufficient because individual difference of arm acceleration due to stature or gait speed were not considered. The objective of this study is to propose the gait classification method based on individual difference. The proposed method classifies three gaits with different foot clearance and step length by machine learning technique using combination of arm acceleration, stature, and gait speed. The proposed method was tested for 1800 steps performed from ten participants. The proposed method could classify three gaits with accuracy greater than 0.9. These results indicate possibility that the proposed method can be used for gait monitoring using wearable device.

It is yet not known whether the variation in knee orthotics pressure would lead to changes in muscle activity during-sit-to-stand postural transition in patients with knee osteoarthritis (OA). Participants in this analytical study were patients with knee OA. The research design was a cross-sectional study. They were enrolled in the study through a sample of convenience method. The primary outcome measure was surface electromyography for measuring muscle activity while changing knee orthotics pressure during sit-to-stand motion. Data were summarized with mean and standard devia-tion while Friedman’s test was performed for multiple comparison of variables, at a significance level of p = 0.05. Seven elderly patients with knee osteoarthritis (mean age 71.4 ± 11.8 years) participated in the study. Moderate orthotics (7.3 mmHg) led to a significant increase in the percentage maximum voluntary contraction (MVC) of tibialis anterior compared to that obtained without orthotics. Rectus femoris, vastus medialis, vastus lateralis, and biceps femoris tended to increase the % MVC with an increase in wearing pressure. It was therefore concluded that the muscle activity during sit-to-stand motion could be increased in patients with knee osteoarthritis by wearing flexible orthotics with varying pressure.

Arteriosclerosis requires early detection and treatment. However, its treatment remains insufficiently established. Elucidating the mechanisms of vasorelaxation may lead to early treatment and contribute to the prevention of diseases caused by arteriosclerosis. The ability of acupuncture/moxibustion stimulation to temporarily increase vasorelaxation and vascular flow has been extensively reported, but its mechanism remains unknown. We successfully modeled the mechanism starting from acupuncture/moxibustion stimulation to vasorelaxation/vasodilation by collating the results of numerous studies in various fields, such as medicine, biology, neurosciences, and pharmacology. Ion channels, including transient receptor potential (TRP) channels, exist at free nerve endings. Acupuncture/moxibustion stimulation is sensed by its ion channels and generates nitric oxide in three subsequent pathways, which leads to vasorelaxation. The three pathways are 1) the nNOS activity pathway (based on neural information), 2) eNOS activity pathway (based on vascular wall shear stress), and 3) iNOS activity pathway (based on multiple cytokines produced by macrophages). These pathways differ depending on the external stimulus; however, in each case, they lead to vasorelaxation and contribute to arteriosclerosis prevention. These pathways can be expected to create complex circuits and produce sustained stimulation effects. This model is expected to contribute to the expansion of the research fields and improvement of treatment methods for medical professionals.

Assistive motion for sit-to-stand causes lower back pain (LBP) among caregivers. Considering previous studies that showed that foot position adjustment could reduce lumbar load during assistive motion for sit-to-stand, quantitative monitoring of and instructions on foot position could contribute toward reducing LBP among caregivers. The present study proposes and evaluates a new method for the quantitative measurement of foot position during assistive motion for sit-to-stand using a few wearable sensors that are not limited to the measurement area. The proposed method measures quantitative foot position (anteroposterior and mediolateral distance between both feet) through a machine learning technique using features obtained from only a single inertial sensor on the trunk and shoe-type force sensors. During the experiment, the accuracy of the proposed method was investigated by comparing the obtained values with those from an optical motion capture system. The results showed that the proposed method produced only minor errors (less than 6.5% of body height) when measuring foot position during assistive motion for sit-to-stand. Furthermore, Bland–Altman plots suggested no fixed errors between the proposed method and the optical motion capture system. These results suggest that the proposed method could be utilized for measuring foot position during assistive motion for sit-to-stand.

Caregivers have lower back pain (LBP) since they must reposition patients in bed frequently. Thus, the low lumbar load posture for turning patients should be explored. In this study, we focused on foot position because it can be easily adjusted to reduce back pain. The hypothesis was that short anteroposterior foot distance could reduce lumbar loads because closer position to patient made smaller moments. Therefore, this study aimed to investigate the relationship between foot position and lumbar loads while turning patients on beds. Furthermore, we compared compression stresses of L4–L5 via computational simulation and erector spinae muscle activities obtained from electromyography (EMG) in nine foot positions. The results showed that short anteroposterior foot distance reduced lumbar loads while turning a patient on a bed.

Knee joint angle sensing in healthy young adults using flexible orthosis with different wearing pressure 査読有り国際誌

[Purpose] To investigate the effects of a flexible brace, by analyzing whether its usage; the difference in wearing pressure could change the joint position sensation in healthy participants; and develop a flexible knee brace for patients with knee osteoarthritis. [Participants and Methods] The study included eight healthy males with 14 knee joints (mean age, 22.0 ± 3.1 years). To measure joint position sense, an "angle reproduction test" was performed in three experimental conditions: 1) participants not wearing the brace, 2) the brace was secured with an appropriate force, and 3) the brace was fully secured using hook-and-loop fasteners. [Results] No significant difference was observed among groups comprising of those not wearing, those wearing with the standard force, and those wearing with the tight force. When the maximum hook-and-loop fastener was squeezed, the sensory error in joint position was maximized at both 30° and 60° flexion. [Conclusion] Joint position sense improvement was confirmed to be poor by orthosis, and an error occurred in the joint position sense by increasing the wearing pressure in orthosis. In future, measurements should be performed on patients with knee osteoarthritis who have decreased joint position sense and verify the effect of different wearing pressures verified.

Because caregivers often experience lower back pain caused by lumbar load from patient handling, monitoring this load can help prevent pain. Erector spinae muscle activity, which is measured and monitored as lumbar load, is commonly measured by electromyography (EMG). However, EMG's electrodes can cause skin irritation and be uncomfortable. Therefore, measuring muscle activity without electrodes is necessary. In this study, we propose a method for estimating erector spinae muscle activity using wearable sensors, specifically inertial and shoe-type force sensors. Inertial sensors measure acceleration and angular velocity of the trunk. Shoe-type force sensors measure vertical force of the feet. A regression model obtained from a machine learning algorithm can predict erector spinae muscle activity using inertial and force data. In our experiment, we evaluated the accuracy of our method by comparing sensor data with surface EMG data in patient handling. Results show that this method can measure erector spinae muscle activity with a small error (less than 5% maximal voluntary contractions) and a significantly high correlation with actual value (r = 0.891, p <0.05). In addition, a Bland-Altman plot showed no fixed and proportional errors. These findings indicate that our proposed method can accurately monitor the lumbar loads of caregivers.

Caregivers have low back pains due to patient handling. Therefore, we have been developing a monitoring system for patient handling designed to prevent low back pains. In this study, we propose a recognition method for patient handling using inertial measurement unit (IMU) and insole pressure sensor for a wearable monitoring system. Our proposed method recognizes recommended body mechanics-based patient handling by machine learning with trunk angle and ground reaction force on feet. Accuracy of the proposed method was evaluated by the experiment in a laboratory environment. Furthermore, accuracies of 7 common algorithms for machine learning are compared for consideration of the most suitable algorithm for the proposed method. Selected 7 algorithms were artificial neural network (ANN), decision tree (DT), k-nearest neighbor (KNN), logistic, random forest (RF), naive Bayes (NB), and support vector machine (SVM). The study participants were 5 young males. Each participant performed a stand assist motion with 2 conditions (natural and body mechanics) 10 trials for each condition. Accuracies were calculated by 10-fold cross validation with 100 data. The results suggested that our proposed method could recognize 2 stand assist motions with high accuracy more than 0.9 of the time in 3 machine learning algorithms. Furthermore, the best algorithm (RF) could recognize 2 stand assist motions with 0.97 accuracy. These results indicated that the proposed method could recognize patient handling motion and could be applied to a monitoring system to prevent low back pain among caregivers. In addition, this study found the best algorithm for patient handling recognition.

Walking canes provide a suitable placement for inertial gait analysis systems such as gait velocity estimation. However, previous velocity estimation methods such as the Zero Velocity Update (ZUPT) method, which are designed for foot mounted inertial sensor systems, are not suitable for walking canes. In this study, we propose the use of a modified velocity update method based on the inverted pendulum model to improve the gait velocity estimation of cane users. The results showed that the proposed method resulted in lower Root Mean Square Error (RMSE) values than the ZUPT method for the estimation of the cane velocity, but the results were worse than previous foot mounted systems.

Background: Falls are a significant incident among older adults affecting one in every three individuals aged 65 and over. Fall risk increases with age and other factors, namely instability. Recent studies on the use of fall detection devices in the Malaysian community are scarce, despite the necessity to use them. Therefore, this study aimed to investigate the association between the prevalence of falls with instability. This study also presents a survey that explores older adults' perceptions and expectations toward fall detection devices. Methods: A cross-sectional survey was conducted involving 336 community-dwelling older adults aged 50 years and older; based on randomly selected participants. Data were analyzed using quantitative descriptive analysis. Chi-square test was conducted to investigate the associations between self-reported falls with instability, demographic and walking characteristics. Additionally, older adults' perceptions and expectations concerning the use of fall detection devices in their daily lives were explored. Results: The prevalence of falls was 28.9%, where one-quarter of older adults fell at least once in the past 6 months. Participants aged 70 years and older have a higher fall percentage than other groups. The prevalence of falls was significantly associated with instability, age, and walking characteristics. Around 70% of the participants reported having instability issues, of which over half of them fell at least once within 6 months. Almost 65% of the participants have a definite interest in using a fall detection device. Survey results revealed that the most expected features for a fall detection device include: user-friendly, followed by affordably priced, and accurate. Conclusions: The prevalence of falls in community-dwelling older adults is significantly associated with instability. Positive perceptions and informative expectations will be used to develop an enhanced fall detection incorporating balance monitoring system. Our findings demonstrate the need to extend the fall detection device features aiming for fall prevention intervention.

〔目的〕軟性膝装具の装着圧力は，動作時に変化せず固定が不十分となるため，動作時に常に固定が得られるように，動的に装着圧力が変化する軟性膝装具が必要となる．本研究はそのような装具開発の基礎研究として，立ち上がり動作時の装着圧力の時間的変化を調べた．〔対象と方法〕対象は，健常若年男性18名とした．異なる装着圧力の軟性膝装具を使用した場合の立ち上がり動作時の装着圧力の時間的変化を調べた．〔結果〕立ち上がり動作の各相における装着圧力の変化において有意差が認められ，殿部離床時に装着圧力が最も大きくなった．また，膝関節屈曲角度に応じて装着圧力が高くなる傾向があった．〔結論〕立ち上がり動作時の軟性膝装具の装着圧力の変化および指標が確認できた．今後は，動的に装着圧力が変化する軟性膝装具の開発を進めていく．

Monitoring of lumbar muscle activity during patient handling is important for the preventing lower back pain among caregivers. Electromyography is used for the measurement of muscle activity; however, EMG has some uncomfortable. Therefore, our previous study proposed measurement method for lumbar muscle activity using wearable sensors and found that the combination of wearable sensors and machine learning can be applied to estimate muscle activity. However, optimal machine learning-based predictor has not been explored. In addition, our previous method requires multiple sensors. Thus, this study explores optimal predictors to estimate lumbar muscle activity during patient handling by a single inertial sensor. The proposed method estimates lumbar muscle activity by acceleration and angular velocity on a trunk and machine learning-based predictor. In the evaluation, five machine learning-based predictors (artificial neural network, gaussian process, k-nearest neighbor, linear regression, and support vector machine) estimated lumbar muscle activity during patient handling. Mean absolute error values of lumbar muscle activity between each predictor and EMG were obtained as actual values. Results show that the proposed method using k-nearest neighbor could estimate muscle activity with minimal error. These results suggest that the proposed method using k-nearest neighbor as an optimal predictor can measure lumbar muscle activity.

Pointing devices commonly used today, including the modern computer mouse, can only be operated manually. For people with physical impairments, usage can be problematic due to a limited ability to operate such devices. Therefore, this study was inspired to conduct research on designing and evaluating an appropriate mouse-substitution system for individuals who are physically impaired. The design of the system uses an inertial measurement unit (IMU) that is a fusion of a gyroscope and an accelerometer sensor in which the sensor is attached to a user's upper arm to recognise physical gestures. Any gestures performed by the upper arm are then mapped and used to manipulate mouse cursor movements on computer devices. The design of the “clicking” method uses both an electromyograph (EMG) sensor and a bend sensor. This study evaluates two input devices; one is a combination of an IMU and an EMG sensor, and the other is an input device that is a combination of an IMU and a bend sensor. Fitts' Law formula and the ISO/TS 9241-411: Ergonomics of human-system interaction standard were used to evaluate quantitative performance and level of comfort. The quantitative results show that the average throughput (TP) of the first input device (2.30 bps) differs greatly, statistically, in comparison to the second input device (1.75 bps). Similarly, the average movement time (tm) revealed that there is a statistically significant difference between the first input device (1.98 s) and the second input device (2.67 s). The qualitative results show that the comfort levels of the first input device are superior to those of the second input device. It concludes that the combination of IMU and EMG as a pointing and clicking apparatus revealed better performance than the combination of IMU and bend sensor.

Caregivers experience lower back pain due to their awkward postures while handling patients. Therefore, a monitoring system to supervise caregivers’ postures using wearable sensors is being developed. This study proposed a postural recognition method for caregivers during postural change while handling a patient on a bed. The proposed method recognizes foot positions and arm movements by a machine learning algorithm using inertial data on the trunk and foot pressure data obtained from wearable sensors. An experiment was conducted to evaluate whether the proposed method could recognize three foot positions and three arm movements. Participants provided postural change for a simulated patient on a bed (patient: supine to lateral recumbent) under nine conditions, including different combinations of the three foot positions and three arm movements; the experiment was repeated ten times for each condition. Experimental results showed that the proposed method using an artificial neural network with all features obtained from an inertial measurement unit and insole pressure sensors could recognize arm movements and foot positions with an accuracy of approximately 0.75 and 0.97, respectively. These results suggest that the proposed method can be used in a monitoring system tracking a caregiver’s posture.

Balance and mobility impairment are major indicators of fall risk in this group. The objective of this research was to develop a fall risk feedback system that operates in real time using an inertial sensor-based instrumented cane. Based on inertial sensor data, the proposed system estimates the kinematics (contact phase and orientation) of the cane. First, the contact phase of the cane was estimated by a convolutional neural network. Next, various algorithms for the cane orientation estimation were compared and validated using an optical motion capture system. The proposed cane contact phase prediction model achieved higher accuracy than the previous models. In the cane orientation estimation, the Madgwick filter yielded the best results overall. Finally, the proposed system was able to estimate both the contact phase and orientation in real time in a single-board computer.

Trans-radial prosthesis is a wearable device that intends to help amputees under the elbow to replace the function of the missing anatomical segment that resembles an actual human hand. However, there are some challenging aspects faced mainly on the robot hand structural design itself. Improvements are needed as this is closely related to structure efficiency. This paper proposes a robot hand structure with improved features (four-bar linkage mechanism) to overcome the deficiency of using the cable-driven actuated mechanism that leads to less structure durability and inaccurate motion range. Our proposed robot hand structure also took into account the existing design problems such as bulky structure, unindividual actuated finger, incomplete fingers and a lack of finger joints compared to the actual finger in its design. This paper presents the improvements achieved by applying the proposed design such as the use of a four-bar linkage mechanism instead of using the cable-driven mechanism, the size of an average human hand, five-fingers with completed joints where each finger is moved by motor individually, joint protection using a mechanical stopper, detachable finger structure from the palm frame, a structure that has sufficient durability for everyday use and an easy to fabricate structure using 3D printing technology. The four-bar linkage mechanism is the use of the solid linkage that connects the actuator with the structure to allow the structure to move. The durability was investigated using static analysis simulation. The structural details and simulation results were validated through motion capture analysis and load test. The motion analyses towards the 3D printed robot structure show 70%–98% similar motion range capability to the designed structure in the CAD software, and it can withstand up to 1.6 kg load in the simulation and the real test. The improved robot hand structure with optimum durability for prosthetic uses was successfully developed.

Many caregivers experience occupational injuries due to patient care activities. An analysis of actual occupational injury reports will be effective to prevent such occupational injuries. However, analyzing these reports is difficult because they comprise text data that have not been quantified or standardized. In this study, we employ a text mining technique and analyze 422 occupational injury reports that include the word “patient.” We obtained Severe Injury Reports published by the Occupational Safety and Health Administration. These reports were investigated using cluster analysis and a co-occurrence network based on Jaccard coefficients. The cluster analysis results indicate that are fall accidents could be caused by cords on the floor, finger injuries were associated with doors, and lower back, or neck pain was related to lifting in patient care activities. The generated co-occurrence network showed that fall accidents and lower back injury are common occupational injuries among caregivers. These results indicate the need to prevent fall accidents and lower back pain among caregivers by improving the work environment and workspace strategies, such as cord placement and caregiver postures.

In our previous research, we used a millimeter wave radar to estimate the position of the feet for fall risk assessment of cane users. Radar sensors have a good accuracy, however, due to its low resolution, it is difficult to know if the radar is really tracking the position of the feet or of any other object. In this research, we present a shoe image detector using SSD-MobileNet architecture that could be used in combination with the radar to accurately track the position of the feet. The results show that the proposed detector could correctly recognize the position of the shoes in an image.

The ideal respiratory sound sensor exhibits high sensitivity, wide-band frequency characteristics, and excellent anti-noise properties. We investigated the body-conducted sound sensor (BCS) and verified its usefulness in respiratory sound monitoring through comparison with an air-coupled microphone (ACM) and acceleration sensor (B & K: 8001). We conducted four experiments for comparison: 1) estimation by equivalent circuit model of sensors and measurement by a sensitivity evaluation system; 2) measurement of tissue-borne sensitivity-to-air-noise sensitivity ratio (SRTA); 3) respiratory sound measurement through a simulator; and 4) actual respiratory sound measurement using human subjects. For 1), the simulation and measured values of all the sensors showed good agreement; BCS demonstrated sensitivity ~10 dB higher than ACM and higher sensitivity in the high-frequency segments compared with 8001. In 2), BCS showed high SRTA in the 600–1000 and 1200–2000-Hz frequency segments. In 3), BCS detected wheezes in the high-frequency segments of the respiratory sound. Finally, in 4), the sensors showed similar characteristics and features in the high-frequency segments as the simulators, where typical breathing sound detection was possible. BCS displayed a higher sensitivity and anti-noise property in high-frequency segments compared with the other sensors and is a useful respiratory sound sensor.

[Purpose] A goniometer is frequently used for static measurement of hindfoot alignment. However, although goniometer measurements require experience, their reliability and validity remain controversial. We developed a hindfoot alignment measurement method by laser as an alternative measure. The purpose of this study was to examine the reliability of laser-assisted hindfoot alignment evaluation. [Participants and Methods] Two non-expert examiners (without medical knowledge), briefly trained in the use of laser-assisted hindfoot alignment evaluation, evaluated hindfoot alignment in 12 healthy participants. [Results] The ICC of the intra-rater reliability was 0.74 for both examiners and the ICC for inter-rater reliability was 0.43. [Conclusion] The good intra-rater and moderate inter-rater reliability of laser-assisted hindfoot alignment evaluation, when used by non-professionals, suggest the laser-assisted hindfoot alignment evaluation may be appropriate for use in clinical practice settings.

© 2020 Mitsumasa Hida et al. Catastrophic thinking is related to pain intensity and the degree of disability and influences pain care significantly. However, only few studies have investigated the impact of catastrophic thinking on chronic pain (CP) in the community-dwelling elderly population. This study aimed to evaluate the characteristics of CP in the community-dwelling elderly population and to investigate the effects of different periods of CP on cognitive and psychological functions. A total of 187 community-dwelling elderly people met the inclusion criteria and were included in this cross-sectional study. The survey items included demographic data (age and gender), pain-related questionnaires, psychological and cognitive functions, and sleep status. The duration of CP was investigated using three categories: no pain and pain for ≤1 year and ≥1 year. A logistic regression analysis was performed to identify the factors most strongly associated with the presence of CP. The difference in each assessment was compared according to duration of CP among the three groups and analyzed using the chi-square test, Kruskal-Wallis test, and one-way analysis of variance. The PCS scores and depression scores were significantly higher in long duration of CP compared with no pain and pain for ≤1 year. The present study is consistent with the fear-avoidance model and was concluded that community-dwelling elderly people with CP are depressive and tend to magnify their pain with long duration of CP.

厚生労働省が定めた「第 13 次労働災害防止計画」では，労働災害の件数が減少していない業種等への対策を推進しており，その対象の1つである社会福祉施設では「動作の反動・無理な動作」による労働災害が多い．そこで本研究では，これらの労働災害におけるリスク要因の抽出を目的として，厚生労働省が公開している「労働災害データベース」に収録された計1041例の労働災害事例を調査した．本研究では労働災害事例の発生状況に関する文章記述を対象とし，これらのテキスト情報をテキストマイニング解析によって調査した．テキストマイニング解析では，各頻出語の頻出回数と頻出語同士の共起関係を示す共起ネットワークによって多くの労働災害事例に共通する具体的な状況を考察した．解析結果は「腰」，「ベッド」，「介助」，「痛み」，「移乗」，「患者」，「車椅子」などの語句が多くの事例で頻出，共起していることを示した．この結果から，対象の労働災害事例の多くにベッドや車椅子間の移乗介助による腰痛が発生している可能性が示された．移乗介助による腰痛は平成20年に実施された先行調査においても指摘されているが，本研究の結果を踏まえるとこれらのリスク要因は未だに解決されていないと考えられる．そのため，今後の労働災害対策では移乗介助による腰痛の予防に向けて従来よりも実効性のある解決策を早急に検討する必要があると考える．

Usefulness of a simulated experience method for transfer assistance for hemiplegia or limited range of motion in multiple joints 査読有り

[Purpose] To further the understanding of dysfunctions to which a simulated experience method could be applied, we clarified whether a simulated experience method can promote caregivers to utilize the abilities of care recipients with pseudo-hemiplegia or pseudo-limited range of motion (ROM) in multiple joints. [Participants and Methods] We studied transfer assistance in 60 nursing assistants from nursing home settings: 30 were assigned to the pseudo-hemiplegia (26 females, 4 males) and limited ROM in multiple joints (27 females, 3 males) groups. One healthy person was fitted with orthotic braces to mimic hemiplegia or limited ROM in multiple joints, each making it difficult to stand-up. Participants were randomized to either a simulated experience group (involving interventional help from a physical therapist between the first and second measurements) or a control group. The load difference on the lower limbs of the care recipient between two rounds of transfer assistance was examined. [Results] The difference between the second and first measurements was −5.9 ± 74.5 N for the control group and 107.9 ± 123.6 N for the simulated experience method in the pseudo-hemiplegia study, and −14.7 ± 64.7 N and 149.1 ± 132.4 N, respectively, for the pseudo-limited ROM-in-multiple-joints study. [Conclusion] The simulated experience method promoted transfer assistance of a care recipient with pseudo-hemiplegia or pseudo-limited ROM in multiple joints. These results suggest that hemiplegia and limited ROM in multiple joints are added as dysfunctions that can be applied to a simulated experience method in transfer assistance.

Optimal foot-position of caregiver based on muscle activity of lower back and lower limb while providing sit-to-stand support 査読有り

[Purpose] In caregivers, low load posture is necessary to prevent lower back pain during patient handling activities such as sit-to-stand support. This study focused on the foot-position of caregivers as an adjustable and useful parameter. A wide stance decreases the stress on the lumbar vertebra. However, this foot-position increases loading of the spinae erector muscles. The aim of this study was to investigate the relationship of anterior-posterior and lateral-medial distances between feet and activity of the spinae erector muscles to determine the optimal foot-position for reducing stress on the lumbar vertebra without increasing spinae erector muscle load. [Participants and Methods] Five young male participants were asked to provide sit-to-stand support 10 times using nine normalized foot-positions with different anterior-posterior and lateral-medial distances. Surface electromyograms of the erector spinae and lower limb muscles were measured during sit-to-stand support. [Results] The results showed that the optimal foot-position (anterior-posterior 55%, lateral-medial 20% of body height) increased muscle activity within the lower limb muscles compared with the lower back muscles and did not increase loads on the erector spinae muscle. [Conclusion] Optimizing foot-position can reduce stress on the lumbar vertebra without increasing load on the spinae erector muscles.

We verified that arteriovenous fistula (AVF) function can be evaluated from shunt sound using a body-conducted sound sensor (BCS) with a wide-band frequency characteristic. In the BCS, we compared two types of sensors: an air-coupled microphone (ACM) and an acceleration sensor (Type 8001). Evaluation for sensors were performed for 36 patients. We also developed a shunt-sound observation system and evaluated it using the spectrogram and power spectrum values obtained using a sound spectrometer. Furthermore, we verified that two indices of ultrasonography information, namely, the resistance index (RI) and flow volume (FV), can be estimated from the shunt sound and classified through machine learning. The BCS detected the shunt sound signal in the spectrogram over a wide frequency band. The sensitivities of the BCS in the non-amplified detection of stenosis sound signals were 0.93/0.92 (below 1 kHz/above 1 kHz, respectively), surpassing those of other sensors(ACM: 0.82/0.77, Type 8001: 0.82/0.25). Furthermore, the highest machine learning accuracy was obtained using BCS in the classification segments of RI > 0.60, 0.65, and 0.70 and FV < 350 ml/min. BCS is particularly suitable for detecting stenosis sounds of over 1 kHz and estimating RI and is considered to be suitable for functional evaluations of AVF.

Background: A device that can measure posture alignment repeatedly is important for the prevention of hyperkyphosis. Objective: We devised a markerless measurement method for hyperkyphosis using digital photography and investigated the correlation with other noninvasive measurements and the validity and accuracy of this method. Methods: The study included 27 participants. The craniovertebral angle in supine (CVAS) and craniohorizontal angle in supine (CHAS) were calculated from digital photographs of the head and neck areas of the studied subjects with ImageJ. The correlations of CVAS and CHAS with the kyphosis index (KI) and block method (BM) were investigated. Intrarater correlation coefficient and Bland–Altman analyses were used to verify the reliability and accuracy of the measured results. Results: CHAS exhibited an excellent correlation with the KI and the BM. The intra- and interrater reliabilities of CHAS were almost perfect. Bland–Altman analysis revealed that CHAS was associated with minor addition errors. Conclusion: CHAS founded an excellent correlation and reliability with the conventional spinal postural alignment measurements. The addition error suggested that the manual was needed to confirm the landmark. The CHAS is a method used to measure the spinal postural alignment in a supine position without markers and without exposing the skin surface.

Caregivers experience low back pain because of patient handling such as supporting standing-up. The lumbar load of a caregiver depends on the required force for patient handling motions. If the relationship between the required force and the lumbar load is quantitatively clarified, it may be useful for preventing low back pain in caregivers. In this study, we investigated the quantitative relationships between the required force and lumbar loads such as vertebral stress and muscle activity in supporting standing-up by computational musculoskeletal simulation. First, a musculoskeletal model of a caregiver was prepared, and then the model performed simulated supporting standing-up motions. The vertical load used as the required force was placed on the upper limb of the model. The compressive/shear stress of the vertebral (L4–L5) and muscle activities of spinae erector muscle group were recorded as the lumbar load. The results showed that there are highly significant correlations between the required force (r > 0.9, p < 0.01). In addition, regression equations for predicting each lumbar load by the required force with highly determination coefficients (R > 0.9) were obtained from these relationships. Furthermore, we found that when the required force was more than 120 N, the compression stresses of the vertebral exceeded injury threshold (3400 N) by the regression equation. These regression equations contribute to quantitatively consider lumbar loads of caregiver during patient handling based on injury thresholds and the required force. 2

It is estimated that one in three seniors fall at least once a year. Falls are a global problem for the elderly that affects their quality of life and poses a great risk. In our research, we are trying to develop a system that could prevent falls by estimating the fall risk in real time. The system would measure the balance of the user by measuring the position of the Center of Gravity inside the Base of Support. In our previous research, we presented a system with a millimeter wave radar attached to a cane to measure the area of the Base of Support. However, the obtained results for the foot position estimation error were significantly worse than similar studies. One of the reasons was that the sensor was not really estimating the position of the feet but the position of the lower legs. Therefore, in this research we present a correction model to improve the feet position estimation. The proposed model was able to reduce the foot position estimation RMSE from 54 mm down to 34 mm, which is closer to the results of other similar studies measuring the position of the feet.

© 2019 IEEE. In order to improve an application from the viewpoint of helping blind people waiting for the bus, classification of the vanishing view is required. The vanishing point from a perspective view is a feature that may improve the performance of viewpoint classification, especially in the case of congested traffic. The idea proposed by this study bases on the vanishing point estimation following car distribution on the road. In essence, the cars are detected using a You Only Look Once (YOLO) technique which extracts all the to the center point of a squared boundary. Normalization of the data points are then computed in percentages. Next is the calculation of the nineteen features of the data points of the car distribution. Finally, the vanishing point is estimated by supervised regression. This experiment compares the estimated results among five different regressions consisting of Linear, k-Nearest Neighbor, Decision Tree, Support Vector, and Multi-layer Per-ceptron regression. The obtain results show that the k-Nearest Neighbor regression computed the lowest distance error of 10.53, compared to the other regression algorithms.

© Published under licence by IOP Publishing Ltd. Some disabilities caused by sensory malfunction, accident, or congenital disorder could cause people difficulty in grasping the computer mouse. A well-known substitute uses an inertial sensor to monitor the body's orientation. This paper evaluates the efficiency and effectiveness of different methods of holding the sensor. There are two modes of holding the sensor, the grasping mode and the attached mode; we also evaluate a mouse as a baseline. The attached mode works by placing the sensor on the back of user's hand. The quantitative and qualitative evaluation procedure is based on ISO/TS 9241 part 411: Ergonomics of human-system interaction standard. The test consists of four levels of difficulty and indicates that the throughput and task completion times were not statistically different between grasping and attached mode. We also found that grasping mode and attached mode did not show significantly statistical differences in comfort and fatigue based on the questionnaires. The results also suggest that the orientation of the hand used in grasping and attached modes is suitable only for lower-impact computer use.

© 2019 World Scientific Publishing Company. Most caregivers have low back pain which results from frequent care activities such as assistance motion that supports transfer and standing-up. Various parameters are associated with the caregiver's lumbar load. In this study, we focus on the foot position of the caregiver as one of the subjective adjustable parameters. This study aimed to analyze the relationship between foot position and stresses of the L4-L5 joint as lumbar load during supporting standing-up via musculoskeletal simulation. The musculoskeletal model was tasked with simulating supported standing-up motions based on a specific pelvic position and angular variation of each joint. The anterior foot (left foot) was fixed, and the posterior foot (right foot) was moved to three backward positions and three rightward positions, thus obtaining nine posterior foot positions. Compressive, anteroposterior shear, and lateral shear stresses of the L4-L5 joint were compared for nine foot positions. The results showed that as the anteroposterior distance and lateral widths between both feet increased, the average value of compressive/shear stress of the L4-L5 joint during motions decreased. From our findings, we hypothesized that the foot position may reduce the lumbar load and prevent low back pain.

© 2019 IEEE. Base of Support (BoS) is a significant parameter for balance analysis. It is particularly important for people using assistive devices such as canes or walkers, because assistive devices improve the stability of the user by increasing the Base of Support. The objective of this research is to develop a wearable device that is able to analyze balance by measuring the position of the Center of Gravity (CoG) inside the Base of Support while using a cane. In this paper, we present our initial work to measure the Base of Support. We developed an instrumented cane with a millimeter wave radar in order to measure the Base of Support in real time. The point cloud data from the radar were grouped into clusters to identify the points belonging to each foot. A density-based spatial clustering of applications with noise (DBSCAN) algorithm was used for this purpose. Finally, the right and left foot were differentiated based on the position of the centroid of each cluster. Knowing the position of both feet with respect to the cane, we were able to measure the BoS with an error of 92.04 cm2(7.38 %).

This paper examines the new study of hand orientation as a substitute for computer-mouse movement and is evaluated based on ISO/TS 9241 part 411: Ergonomics of human-system interaction-evaluation methods for the design of physical input devices. Two pairs of hand-orientation candidates were evaluated, using, for example, pitch-roll and pitch-yaw to substitute for up-down and left-right mouse-cursor movements. The up-down cursor movement was generated from the pitch orientation, while the left-right cursor movement was generated from the roll or yaw orientation, depending on the evaluation of the proposed gesture. The research employed a standard computer mouse as a baseline comparison for the study. The empirical study was conducted to evaluate quantitative performance such as throughput and movement time. The best impression resulted when the throughput had the greatest value as well as the shortest movement time. The performance test was based on Fitts's law using a multi-directional tapping test as suggested by ISO/TS 9241-411. The test was divided into several levels of difficulty, including high, medium, low, and very low. The other assessment is qualitative and was performed using the comfort-rating scale questionnaire and rating of perceived exertion of comfortability and fatigue. The quantitative results show that pitch-yaw throughput is slightly higher than for the pitch-roll gesture, and that the movement time in pitch-yaw is slightly less than in pitch-roll, although there is no statistically significant difference between the two. We also found that pitch-yaw movements have a higher level of comfort based on the comfort-rating scale test. Since the test was divided into levels of difficulty, we identified those gestures suitable for the task with a low and very low level of difficulty based on throughput, movement time, and error-rate results. Finally, this study suggests that pitch-roll and pitch-yaw movements of the hand can be used as substitutes for the mouse, and that pitch-yaw movements are superior in regard to causing less fatigue than pitch-roll movements. Furthermore, this study provides a new suggestion for a suitable level of difficulty when using an inertial sensor as an emulator for the movement of a mouse cursor in the field of human-computer interaction.

© 2018 IEEE. A smart phone application capable of recognizing numbers of oncoming buses would be of great assistance to blind individuals. To facilitate bus route number reading, obstacles along the road should first be identified. This paper is concerned with identification of static obstacles comprising two processes: the first process involves road area detection and is addressed by applying a rotational invariant of the uniform local binary pattern via k-means clustering. Furthermore, an artificial neural network is employed to select a group of k-means that contains the road area. Next, the straight lines on the road are detected via Hough line transformation. Finally, the line selection step is used to define the road area boundary. The second process involves static obstacle detection and is addressed through segmentation, vertical projection of the road area boundary, and consideration of the vertically projected signal. The experimental results demonstrate a high performance of the proposed method with an F-measure of 0.912.

© Springer Nature Switzerland AG 2019. We investigated the effect of age on heart rate (HR) responses and subjective mental workload during mental tasks. In this study, 55 male participants performed a mental arithmetic (MA) task and a mirror tracing (MT) task for 5 min each. Electrocardiogram (ECG) was recorded during these mental tasks and resting periods. Low frequency component (LF), high frequency component (HF), and LF/HF ratio of heart rate variability (HRV) were derived from ECG signals. NASA Task Load Index (NASA-TLX) was used to evaluate the subjective mental workload during each task. In the results, HR was significantly larger in the MA task than the resting period. LF was significantly smaller in the MA than the resting period before the task. On the other hand, no HR changes from the baseline in the MT task were found, suggesting that Pattern 2 response might appear. In the results of HRV indices, decrease in LF suggests the inhibition of the parasympathetic nervous system activity by the task. These results are consistent with those of our previous studies. No significant main interaction with age and blocks (MA, MT and resting periods) were found in physiological responses except for LF/HF and subjective mental workload score. Therefore, the physiological responses induced by mental tasks might be more susceptible to task characteristic than to aging. This study partly supported the previous study.

To provide an effective notification service for the blinds awaiting the bus, it is crucial to have a viewpoint classification technique in which a viewpoint is defined with tilt and panning of camera. This paper proposes a viewpoint classification method using the car distribution information in the congested traffic environment. The proposed method takes four steps for classification. First, the YOLO algorithm is used to detect the car positions in the images. Second, the car positions are normalized for feature computation. Third, nineteen simple features are extracted and finally, the viewpoint classification is conducted. The proposed method uses the information gain measure to select relevant ones from the extracted features, and uses the Random Forest algorithm as a classifier. In the experiments, the proposed method has been tested for various roadside scenarios of congested traffic in day and night. The accuracies for car detection and viewpoint classification were 79:90% and 86:00%, respectively, which are improved compared to the prior work.

[Purpose] To clarify the reason why the axillary pad often deviates from the axilla during double-crutch walking using a musculoskeletal simulator. [Participants and Methods] The subjects were five healthy young males without orthopedic disease. Movement during crutch walking was measured using a 3D motion analysis system. Based on the obtained data, muscle activity related to the shoulder joint during double-crutch walking was modeled using a musculoskeletal simulation. [Results] The shoulder joint was abducted in the crutch stance phase of all subjects. [Conclusion] The results suggest that crutch walking with the shoulder blades rotated downward may suppress the activity of the abductor muscle of the shoulder joint and the activity of the shoulder joint adductor muscle.

[Purpose] We aimed to clarify whether demonstration and simulated experience help the ability of care-receivers to get transferred, such as from the bed to the commode. [Participants and Methods] Participants included 28 nurses and 17 caregivers (34 females and 11 males). We developed a total floor reaction force measurement device to quantify the total loading level of care-receivers and caregivers and force shoes to quantify the loading level of the caregivers. Using these instruments, we constructed a system to measure the load on the lower limbs of the care-receivers during partial assistance. We divided the participants into the control, demonstration, and simulated experience method groups. We examined the differences in the load on the lower limbs before and after the intervention. [Results] The loads on the lower limbs of care-receivers when their buttocks were lifted from the chair were 11.7 ± 69.6, 61.8 ± 79.4, and 101.0 ± 104.0 N in the control, demonstration, and simulated experience groups. [Conclusion] These data suggest that the simulated experience method could help make use of the ability of the care-receiver to get transferred. Even care workers for the sanatorium-type sickbeds could learn to utilize the physical ability of the care-receivers using simulated experience.

[Purpose] Caregivers experience low back pain owing to frequent patient handling motions such as supporting the body while standing up. To prevent low back pain in caregivers, low load posture while engaging in patient handling motions is required. We determined the relationship between surface electromyography of the erector spinae muscles and subjective step length as "long" and "short" during the supporting standing-up motions of caregivers. [Participants and Methods] Ten young male participants were asked to perform supporting standing-up motion 10 times using two-step lengths comprised of subjective long and short steps. During supporting standing-up motion, we measured surface electromyograms of the erector spinae muscles and calculated the integral electromyographic values. [Results] The subjective long/short-step length normalized by body height did not differ across the participants. In addition, the subjective long-step length was longer than the subjective short-step length in all the participants. Integral electromyographic values for both the left and right erector spinae muscles in the short-step length were significantly lower than those in the long-step length when the data obtained from all the participants were used. [Conclusion] We considered that the load of the erector spinae muscle will be reduced if the short-step instead of the long-step instruction is given. In the future, instructions based on the subjective step-length variation in caregivers must be considered.

Developing an anthropomorphic robotic hand (ARH) has become a relevant research field due to the need to help the amputees live their life as normal people. However, the current state of research is unsatisfactory, especially in terms of structural design and the robot control method. This paper, which proposes a 3D printed ARH structure that follows the average size of an adult human hand, consists of five fingers with a tendon-driven actuator mechanism embedded in each finger structure. Besides that, the movement capability of the developed 3D printed robot hand validated by using motion capture analysis to ensure the similarity to the expected motion range in structural design is achieved. Its system functionality test was conducted in three stages: (1) muscular activity detection, (2) object detection for individualfinger movement control, and (3)integration ofbothstages in one algorithm. Finally, an ARHwas developed, whichresembles human hand features, as well as a reliable system that can perform opened hand palm and some grasping postures for daily use.

Currently, not only able-bodied people but also people with disabilities may be interested in sports climbing. However, evaluation and guidance in climbing competitions are subjective, and those who attempt sports climbing for the first time may find it difficult to compete. Additionally, the current training methods have been developed for able-bodied people, and it is not known whether they are suitable for people with disabilities. If an objective evaluation method is established, it will be possible to propose a rational and effective teaching method for sports climbing participants. Therefore, the ultimate goal of this study is to develop an objective training method for climbing for lower-leg amputees. We studied subjects climbing a ladder that imitated the wall-climbing process and analyzed the relationship between EMG of the upper limbs and body center acceleration during climbing, which revealed a close relationship.

© 2018 IEEE. In this paper, we propose an educational system of assistance motions using wearable sensors. In addition, we will briefly introduce our proposed system and evaluate the classification method of assistance motion with different foot positions using wearable sensors as a first step toward developing an educational system. Our proposed system comprises shoe-type pressure sensors and a single inertial measurement unit attached to the trunk. The proposed method classifies assistance motion by machine learning technique using feature quantities obtained from wearable sensors. Herein we evaluated whether the proposed method could classify assistance motions for supporting standing-up with different foot positions. Participants were five young men and were asked to perform two motions with different two feet anteroposterior positions positions ("short step length" and "long step length"). The results showed that the proposed method was able to classify the two motions with 90% or higher accuracy for all participants. Therefore, we considered that the method could be used in the educational system.

© 2018 IEEE. In order to provide a notification system for the blind awaiting a bus, a classification of viewpoints is necessary. In a previous study, we classified the viewpoints in non-congested traffic using road features extraction. However, the viewpoints in congested traffic have not yet been discussed; hence this paper proposes a viewpoints classification using car distribution on the road in congested traffic. The study aimed to classify the viewpoints to two classes as "Good Viewpoints" and "Bad Viewpoints" with 100 images for each class. The experimental results showed a 76% accuracy of the proposed method.

This paper presents a novel approach to predicting self-calibration in a pressure sensor using a proposed Levenberg Marquardt Back Propagation Artificial Neural Network (LMBP-ANN) model. The self-calibration algorithm should be able to fix major problems in the pressure sensor such as hysteresis, variation in gain and lack of linearity with high accuracy. The traditional calibration process for this kind of sensor is a time-consuming task because it is usually done through manual and repetitive identification. Furthermore, a traditional computational method is inadequate for solving the problem since it is extremely difficult to resolve the mathematical formula among multiple confounding pressure variables. Accordingly, this paper describes a new self-calibration methodology for nonlinear pressure sensors based on an LMBP-ANN model. The proposed method was achieved using a collected dataset from pressure sensors in real time. The load cell will be used as a reference for measuring the applied force. The proposed method was validated by comparing the output pressure of the trained network with the experimental target pressure (reference). This paper also shows that the proposed model exhibited a remarkable performance than traditional methods with a max mean square error of 0.17325 and an R-value over 0.99 for the total response of training, testing and validation. To verify the proposed model’s capability to build a self-calibration algorithm, the model was tested using an untrained input data set. As a result, the proposed LMBP-ANN model for self-calibration purposes is able to successfully predict the desired pressure over time, even the uncertain behaviour of the pressure sensors due to its material creep. This means that the proposed model overcomes the problems of hysteresis, variation in gain and lack of linearity over time. In return, this can be used to enhance the durability of the grasping mechanism, leading to a more robust and secure grasp for paralyzed hands. Furthermore, the exposed analysis approach in this paper can be a useful methodology for the user to evaluate the performance of any measurement system in a real-time environment.

© 2017 IEEE. This study aimed to decrease risk of falls via displacement of the axilla during crutch walking. An axillary force feedback system is developed for providing a visual information to crutch users. An imbalance point is existed between the force of the upper arm on the axillary pad and the force of the chest wall on the axillary pad. Moreover, the system also provides real-time information on force placed on axillary pads during crutch walking and evaluated the effectiveness of our training system. Results suggested that this feedback system was effective for beginners.

[Purpose] It is clinically important to accurately evaluate the alignment of hindfoot alignment from the body surface in weight-bearing position. In general, measurement is carried out using a goniometer, but the measurement with goniometer requires experience, and the reliability in this measurement is also in question. Therefore, it is necessary to establish a clinical measurement method to replace the goniometer. As a solution, we devised a hindfoot alignment measurement method with laser beam (HAML), which measures hindfoot alignment along the body surface using a laser beam. This study aimed to study the correlation between the measurement result along the body surface with laser or goniometer and that of radiography with regard to hindfoot alignment. [Subjects and Methods] Subjects included 23 individuals with knee osteoarthritis. Their hindfoot alignment using radiography and goniometry were measured. The correlation between the measurement result along the body surface with or without laser and radiography was investigated. [Results] The correlation between HAML and radiography was better than that between HAML and goniometry. [Conclusion] The correlation between radiography and HAML for measuring hindfoot alignment was good. This finding implies that HAML can be used as a screening test or epidemiological survey in regional medicine.

[Purpose] An estimation model of the knee and ankle joint angles during the extension phase was proposed in the previous study. However, it had limited use because of the fixed initial lower limb angle before standing up. This study aimed to propose a new estimation model of the initial lower limb angle to improve the angle estimation during extension phase. [Subjects and Methods] Seven healthy male volunteers were enrolled. The new estimation model approximated the initial lower limb angle using a force sensor plate that measured the plantar pressure of the subjects. The estimated angle and force were compared to those obtained by a motion capture system and force plate. [Results] The new estimation model of initial lower limb angle showed no significant difference compared with the true values obtained by motion capture, except for the subject who had a greater foot-pressure measurement error compared with the force plate measurement, with maximum errors of 5.98° and 6.31°, respectively. [Conclusion] The proposed model in this study can estimate the initial lower limb angle before standing and can be applied to the angle estimation model during the extension phase of the standing-up movement.

[Purpose] Angle measurement using images of bony prominences (AMI) determines frontal plane knee alignment from the skin surface without using radiation. The purpose of this study was to assess the validity and reliability of images obtained with the AMI method. [Participants and Methods] The study included 21 patients with osteoarthritis. We measured the functional axis of the lower limb, obtained via full-leg radiography and correlated the findings with the angle of dissection and the angle measured with the AMI method. Additionally, we assessed the reliability of the AMI method. [Results] The angle obtained using the AMI method and the radiographic anatomic axis (the full-leg radiograph)/the mechanical axis were well correlated. The AMI method also showed high reliability. [Conclusion] The AMI method is a valid and reliable alternative to full-leg radiography for imaging of functional and anatomical knee axes. As AMI does not use special equipment or involve radiation exposure, the method can be used in outside medical facilities and can be repeated over time without increased risk to patients.

Body-conducted sound sensors have an electret condenser microphone with an exposed diaphragm. By covering the sensor with a urethane elastomer, the sensor can be used as a bioacoustic sensor with high sensitivity and resistance to external noise. The body-conducted sound sensors constructed in the past had different parameters such as contact area, thickness of the propagation layer and mass, and the extent of their influence on frequency characteristics is unclear. This study aimed to investigate the influence of the mass and shape of body-conducted sound sensor on pressure sensitivity. The methods comprised ( 1 ) simulation using the equivalent circuit model, ( 2 ) production of body-conducted sound sensors and measurement systems, and ( 3 ) sensitivity measurements and analyses. The simulation results demonstrated that the sensitivity increased when the body-conducted sound sensor had a small mass and a large contact area. In the sensitivity measurement system, a compact acceleration sensor was used as a reference sensor, and the sensitivity to pressure was calculated. The results obtained from the sensitivity measurement system correlated strongly with the frequency characteristics of the simulations, with a correlation coefficient of 0.90-0.95 in the frequency range of 100-2,000 Hz. In the statistical analysis, by varying the mass and contact area, significant differences were observed in the frequency range of 700-2,000 Hz while no significant differences were observed in the frequency range of 100-600 Hz. Moreover, as a result of reducing the housing mass, sensitivity was increased. In conclusion, body-conducted sound sensors with a small mass and large contact area exhibit high pressure sensitivity in the frequency range of 700-2,000 Hz, and their frequency characteristics can be estimated by simulation.

One of the examples is the application to measure muscle response or electrical activity in response to a nerve's stimulation of the muscle. The development of wireless Electromyography (EMG) for remote monitoring of muscle activities is one of many vital parts in the biomedical technology. This wireless EMG is able to transmit signals through wireless transmission to computer for monitoring. By employing this technology, doctors treating patients with muscle complication can monitor their patients from the comfort of their office while the patients roam freely in their own room or therapy room. This technology is essential for patients with needs of home-based monitoring and economical medical services. The design of wireless EMG system comprises of a preamplifier and an electrode for the measurement of EMG signal, main amplifier for signal processing, DSP processor for A/D conversion and X-Bee module for wireless transmission. During power spectral density, EMG signal is distributed between 10 to 500Hz. Thus, EMG signals become consistent with the application of wireless transmission.

© 2017 Universiti Putra Malaysia Press. This paper presents an auto grasping algorithm of a proposed robotic gripper. The purpose is to enhance the grasping mechanism of the gripper. Earlier studies have introduced various methods to enhance the grasping mechanism, but most of the works have not looked at the weight measurement method. Thus, with this algorithm, the weight of the object is calculated based on modified Wheatstone Bridge Circuit (WBC) which is controlled by programmable interface controller (PIC) method. Having this approach introduces and improves the grasping mechanism through an auto grasping algorithm. Experimental results show that an auto grasping algorithm based on pressure sensor measurements leads to a more precise grasping measurement and consequently enhance the sensitivity measurement as well as accurate movement calibration. Furthermore, several different grasping objects based on the proposed method are examined to demonstrate the performance and robustness of our approach.

The findings show that lifting required higher forces compared with grasp force in the static phase.

The aim of this study was to propose an estimation model for knee and ankle joint angle measurements and locate body center of gravity (COG) of the extension phase during standing-up motion. [Subjects and Methods] Seven healthy male volunteers were enrolled. An estimation model was proposed for the knee and ankle joint angle measurements by combining the angle and acceleration of the trunk, based on readings from the inertial sensor attachment on the subject’s chest, during the extension phase. Joint angles and COG position were compared to those obtained by a motion capture system. [Results] The joint angles and COG position demonstrated high correlation coefficients which represent strong correlation between the proposed model and the motion capture system. The proposed model could estimate the joint angle during extension phase, with a maximum error of 4.58 degrees, as well as COG position in the horizontal and vertical directions with maximum errors of 4.48 cm and 3.19 cm, respectively. [Conclusion] The proposed system could be used instead of motion capture system to estimate knee and ankle joint angles; however, the estimation of the COG position was insufficient because of lacked accuracy.

Attitude Estimation using Kalman Filtering: External Acceleration Compensation Considerations 査読有り国際誌

Attitude estimation is often inaccurate during highly dynamic motion due to the external acceleration. This paper proposes extended Kalman filter-based attitude estimation using a new algorithm to overcome the external acceleration. This algorithm is based on an external acceleration compensation model to be used as a modifying parameter in adjusting the measurement noise covariance matrix of the extended Kalman filter. The experiment was conducted to verify the estimation accuracy, that is, one-axis and multiple axes sensor movement. Five approaches were used to test the estimation of the attitude: (1) the KF-based model without compensating for external acceleration, (2) the proposed KF-based model which employs the external acceleration compensation model, (3) the two-step KF using weighted-based switching approach, (4) the KF-based model which uses the threshold-based approach, and (5) the KF-based model which uses the threshold-based approach combined with a softened part approach. The proposed algorithm showed high effectiveness during the one-axis test. When the testing conditions employed multiple axes, the estimation accuracy increased using the proposed approach and exhibited external acceleration rejection at the right timing. The proposed algorithm has fewer parameters that need to be set at the expense of the sharpness of signal edge transition.

This paper presents an optimization of grasping object based on pressure sensor measurement. In this work, a commercial flexi force pressure sensors are assembled on the robotic hand gripper model with introducing new arrangement of pressure distributions. For optimization purpose, the authors introduced Wheatstone Bridge Circuit (WBC) methods whereby the pressure distributions were applied. Therefore, with the new arrangement of pressure sensor distribution it has promised higher sensitivity as well as stability for the developed robotic hand gripper system. This framework is considered as one of the techniques that can be applied for robotic hand applications based on pressure sensor especially for high-risk tasks such as military and dangerous environment. In other word, it can save people's life.

Breathing sound reflects the respiratory status, such as a state of airway, effort of respiration, alteration of sound induced by disease, and so on. We have been developing a breath sound sensor with improved acoustic impedance matching. In this paper, we compared two simulation methods based on the frequency characteristics of sensor. First of all, we executed numerical analysis of breath sound sensor, based on the electrical equivalent circuit model. Next, we used the FEM to simulate compared with the numerical simulation in electrical equivalent circuit model. The reason why to use FEM was that FEM could simulate the various conditions of sensor parameter. From the results, we concluded FEM simulation was more useful.

[Purpose] The aim of this study was to estimate the stability of the axillary pad in the axilla during single-crutch walking through kinematic analysis of the crutch position in the horizontal plane. [Subjects] The subjects were 19 healthy young males. [Methods] The subjects were instructed to walk 5 m in a straight line while using a single crutch. This was repeated 10 times. Repeated-measures analysis of variance and Tukey's honest significance test were used to compare crutch angles across three time points during crutch stance (crutch contact, mid stance and crutch off) for each subject. [Results] A statistically significant effect of time was found in 17 of the 19 subjects. Sixteen of these 17 subjects rotated the crutch externally from crutch contact to crutch off. [Conclusion] External rotation of the crutch throughout the stance phase is important to enable the axillary pad to be held tightly against the chest wall. These results may lead to alterations of the instructions for crutch use and the prevention of falls.

筆者らは,いつでもどこでも歩行訓練が可能な空間を実現するという『u-リハビリ(ユビキタスリハビリ)空間』構想を提唱している.この構想では,(1)日常生活における歩行情報の取得,(2)歩行情報の実時間呈示とそれによる患者自身による客観的な歩行状態の把握を実現する.これにより,従来は定性的であった歩行情報の定量化,リハビリ時の患者への指示の可視化,遠隔リハビリテーションなどを目指している.本論文では,まず歩行情報取得のための靴型計測装置の概要を述べ,臨床での使用結果を踏まえて有効性及び改善課題を整理する.次にその改善課題についてのシステムの改良研究の結果を述べる.臨床使用の結果,開発した歩行訓練システムを使うことで,臨床で用いられている計測装置と同等の歩行情報の取得が可能であること,直線歩行訓練や歩幅調製訓練などを患者一人で繰り返し行うことが可能なことが明らかとなった.また,課題として挙げられた通信デバイスに起因する問題や計測範囲の狭さについて,新しいハードウェア機構やセンサ配置の最適化を行うことで,改良することができた.

For the deaf-blind person, in order to develop communication support device which could handwrite a letter on a palm, we have been studying apparent movement phenomenon by cold stimulus on a palm. From our previous experimental results, it was hypothesized that the reason why the apparent movement was not induced at certain region of palm was difference in perceptive characteristics for cold sensation. Therefore, we proposed new stimulation presentation method to induce apparent movement based on perceptive characteristics for cold sensation. In the first experiment, perceptive characteristics for cold sensation on the palm were investigated. Response time to perceive coldness was used as one perceptive characteristics and that time was measured on 72 regions of palm by using peltier element and thermocouple. As a result, it was found that response time varied with palm region and subject. In the second experiment, an occurrence ratio of apparent movement by using stimulation presenting method based on the response time was compared with that by previous method in which response time was not considered. The difference between stimulation presenting method based on the response time and method without consideration for the response time was that response time was added to stimulus duration in the former method. From the results, it was found the occurrence ratio of apparent movement increased when two cold stimuli were presented by using stimulation presenting method based on the response time at two regions where both of response times were above average.
