[Purpose] To identify the lumbar loading movements necessary in clinical practice. [Participants and Methods] A questionnaire survey was conducted among physical and occupational therapists in Japan. There were no exclusion criteria regarding the number of years of experience, age, or field of employment. The participants were randomly selected and administered the questionnaire. They were asked to list and rank the lumbar loadings they considered necessary. [Results] A total of 739 respondents participated in the survey. The results of this nationwide survey indicated that the lifting movement of heavy objects in the trunk flexion position was the most common movement (for 354 participants). [Conclusion] The main loading movements of the lumbar spine were reported to be heavy lifting movements (in the trunk flexion position) and trunk rotation movements. As perspectives, we aim to conduct an analytical study of some of lumbar spine loading movements outlined in this study, using a musculoskeletal simulator and electromyography.

Supporting the elderly by utilizing their remaining functions will lead to the extension of their healthy life span. In this study, we developed a portable cane-type standing and sitting positional mobility support device. In this study, we developed a cane-type mobility support device, and selected a suitable support direction for elderly people by dynamic analysis simulation. In addition, the necessary assisting force for the elderly was examined and the assisting force was set. The effectiveness of the device was evaluated by measuring the surface electromyogram of healthy subjects. As a result, it was found that the specifications of this device were effective when the assist direction was 30 degrees and the assist force was 110 N. From the measurement of the surface electromyogram, it was found that the device was effective for both standing and sitting motions. It also helps to ensure the safety of the sitting motion of elderly people.

変形性膝関節症患者は、膝装具装着により関節位置覚が改善することが報告されているが、面ファスナーの締め具合を示す装着圧力には着目されていないことが多い。装着により局所圧迫の神経生理学的な効果が得られているため、装着圧力を高めることにより、さらに関節位置覚が改善する可能性がある。そこで、本研究は変形性膝関節症患者を対象に膝装具の装着圧力に着目し、関節位置覚に与える影響を検証した。結果より、膝関節屈曲60°においては、High PressureがControlと比較して有意に低い値を示した（p ＜ 0.05）。軟性膝装具の装着圧力を高くすることにより、関節位置覚が向上する可能性が示唆された。

Identification of the cause of fall during the pre-impact fall period 査読有り国際誌

[Purpose] This study aimed to develop and validate a method for identifying factors that may cause a fall during the pre-impact fall period using wearable sensors. [Participants and Methods] The participants were 23 young people from the public data set (mean age, 23.4 years). Acceleration and angular velocity information obtained from sensors attached to the participant’s waist was used to generate the pre-impact fall. The cause of the fall (slip, trip, fainting, get up, sit down) was then classified with and without the addition of activity of daily living data using three different support vector machine. In addition, we investigated the influence of lead time (0–2.0s) on accuracy. [Results] The quadratic and cubic support vector machine identified the activity of daily living and fall patterns more accurately than the linear support vector machine, and the cubic support vector machine was better for classification, although the difference was slight. The greatest accuracy for predicting the cause of the fall (87.9%) was obtained when the cubic support vector machine was used, activity of daily living was factored into the analysis, and the lead time was 0.25 sec. [Conclusion] Support vector machine can identify the cause of the fall during the pre-impact fall period. Appropriate individualized interventions may be designed based on the most likely cause of fall as identified by this analysis method.

Unsuitable foot clearance and step length lead falling accidents; thus, these gait parameters should be monitored for fall prevention. According to previous study, users prefer wrist as a position to wear wearable device for daily gait monitoring. Therefore, we have been developing gait monitoring method using wearable device on a wrist. Our previous method classifies gaits with different foot clearance or step length by machine learning technique using 3-axis acceleration data obtained from wearable device on a wrist. The results of our previous study showed that this method could classify gaits for each step. However, accuracy of our previous method was insufficient because individual difference of arm acceleration due to stature or gait speed were not considered. The objective of this study is to propose the gait classification method based on individual difference. The proposed method classifies three gaits with different foot clearance and step length by machine learning technique using combination of arm acceleration, stature, and gait speed. The proposed method was tested for 1800 steps performed from ten participants. The proposed method could classify three gaits with accuracy greater than 0.9. These results indicate possibility that the proposed method can be used for gait monitoring using wearable device.

It is yet not known whether the variation in knee orthotics pressure would lead to changes in muscle activity during-sit-to-stand postural transition in patients with knee osteoarthritis (OA). Participants in this analytical study were patients with knee OA. The research design was a cross-sectional study. They were enrolled in the study through a sample of convenience method. The primary outcome measure was surface electromyography for measuring muscle activity while changing knee orthotics pressure during sit-to-stand motion. Data were summarized with mean and standard devia-tion while Friedman’s test was performed for multiple comparison of variables, at a significance level of p = 0.05. Seven elderly patients with knee osteoarthritis (mean age 71.4 ± 11.8 years) participated in the study. Moderate orthotics (7.3 mmHg) led to a significant increase in the percentage maximum voluntary contraction (MVC) of tibialis anterior compared to that obtained without orthotics. Rectus femoris, vastus medialis, vastus lateralis, and biceps femoris tended to increase the % MVC with an increase in wearing pressure. It was therefore concluded that the muscle activity during sit-to-stand motion could be increased in patients with knee osteoarthritis by wearing flexible orthotics with varying pressure.
