Assistive motion for sit-to-stand causes lower back pain (LBP) among caregivers. Considering previous studies that showed that foot position adjustment could reduce lumbar load during assistive motion for sit-to-stand, quantitative monitoring of and instructions on foot position could contribute toward reducing LBP among caregivers. The present study proposes and evaluates a new method for the quantitative measurement of foot position during assistive motion for sit-to-stand using a few wearable sensors that are not limited to the measurement area. The proposed method measures quantitative foot position (anteroposterior and mediolateral distance between both feet) through a machine learning technique using features obtained from only a single inertial sensor on the trunk and shoe-type force sensors. During the experiment, the accuracy of the proposed method was investigated by comparing the obtained values with those from an optical motion capture system. The results showed that the proposed method produced only minor errors (less than 6.5% of body height) when measuring foot position during assistive motion for sit-to-stand. Furthermore, Bland–Altman plots suggested no fixed errors between the proposed method and the optical motion capture system. These results suggest that the proposed method could be utilized for measuring foot position during assistive motion for sit-to-stand.

Caregivers have lower back pain (LBP) since they must reposition patients in bed frequently. Thus, the low lumbar load posture for turning patients should be explored. In this study, we focused on foot position because it can be easily adjusted to reduce back pain. The hypothesis was that short anteroposterior foot distance could reduce lumbar loads because closer position to patient made smaller moments. Therefore, this study aimed to investigate the relationship between foot position and lumbar loads while turning patients on beds. Furthermore, we compared compression stresses of L4–L5 via computational simulation and erector spinae muscle activities obtained from electromyography (EMG) in nine foot positions. The results showed that short anteroposterior foot distance reduced lumbar loads while turning a patient on a bed.

Knee joint angle sensing in healthy young adults using flexible orthosis with different wearing pressure 査読有り国際誌

<p> [Purpose] To investigate the effects of a flexible brace, by analyzing whether its usage; the difference in wearing pressure could change the joint position sensation in healthy participants; and develop a flexible knee brace for patients with knee osteoarthritis. [Participants and Methods] The study included eight healthy males with 14 knee joints (mean age, 22.0 ± 3.1 years). To measure joint position sense, an "angle reproduction test" was performed in three experimental conditions: 1) participants not wearing the brace, 2) the brace was secured with an appropriate force, and 3) the brace was fully secured using hook-and-loop fasteners. [Results] No significant difference was observed among groups comprising of those not wearing, those wearing with the standard force, and those wearing with the tight force. When the maximum hook-and-loop fastener was squeezed, the sensory error in joint position was maximized at both 30° and 60° flexion. [Conclusion] Joint position sense improvement was confirmed to be poor by orthosis, and an error occurred in the joint position sense by increasing the wearing pressure in orthosis. In future, measurements should be performed on patients with knee osteoarthritis who have decreased joint position sense and verify the effect of different wearing pressures verified.</p>

Because caregivers often experience lower back pain caused by lumbar load from patient handling, monitoring this load can help prevent pain. Erector spinae muscle activity, which is measured and monitored as lumbar load, is commonly measured by electromyography (EMG). However, EMG's electrodes can cause skin irritation and be uncomfortable. Therefore, measuring muscle activity without electrodes is necessary. In this study, we propose a method for estimating erector spinae muscle activity using wearable sensors, specifically inertial and shoe-type force sensors. Inertial sensors measure acceleration and angular velocity of the trunk. Shoe-type force sensors measure vertical force of the feet. A regression model obtained from a machine learning algorithm can predict erector spinae muscle activity using inertial and force data. In our experiment, we evaluated the accuracy of our method by comparing sensor data with surface EMG data in patient handling. Results show that this method can measure erector spinae muscle activity with a small error (less than 5% maximal voluntary contractions) and a significantly high correlation with actual value (r = 0.891, p <0.05). In addition, a Bland-Altman plot showed no fixed and proportional errors. These findings indicate that our proposed method can accurately monitor the lumbar loads of caregivers.

Caregivers have low back pains due to patient handling. Therefore, we have been developing a monitoring system for patient handling designed to prevent low back pains. In this study, we propose a recognition method for patient handling using inertial measurement unit (IMU) and insole pressure sensor for a wearable monitoring system. Our proposed method recognizes recommended body mechanics-based patient handling by machine learning with trunk angle and ground reaction force on feet. Accuracy of the proposed method was evaluated by the experiment in a laboratory environment. Furthermore, accuracies of 7 common algorithms for machine learning are compared for consideration of the most suitable algorithm for the proposed method. Selected 7 algorithms were artificial neural network (ANN), decision tree (DT), k-nearest neighbor (KNN), logistic, random forest (RF), naive Bayes (NB), and support vector machine (SVM). The study participants were 5 young males. Each participant performed a stand assist motion with 2 conditions (natural and body mechanics) 10 trials for each condition. Accuracies were calculated by 10-fold cross validation with 100 data. The results suggested that our proposed method could recognize 2 stand assist motions with high accuracy more than 0.9 of the time in 3 machine learning algorithms. Furthermore, the best algorithm (RF) could recognize 2 stand assist motions with 0.97 accuracy. These results indicated that the proposed method could recognize patient handling motion and could be applied to a monitoring system to prevent low back pain among caregivers. In addition, this study found the best algorithm for patient handling recognition.

Walking canes provide a suitable placement for inertial gait analysis systems such as gait velocity estimation. However, previous velocity estimation methods such as the Zero Velocity Update (ZUPT) method, which are designed for foot mounted inertial sensor systems, are not suitable for walking canes. In this study, we propose the use of a modified velocity update method based on the inverted pendulum model to improve the gait velocity estimation of cane users. The results showed that the proposed method resulted in lower Root Mean Square Error (RMSE) values than the ZUPT method for the estimation of the cane velocity, but the results were worse than previous foot mounted systems.

The Association of Falls with Instability: An Analysis of Perceptions and Expectations toward the Use of Fall Detection Devices Among Older Adults in Malaysia 査読有り

Background: Falls are a significant incident among older adults affecting one in every three individuals aged 65 and over. Fall risk increases with age and other factors, namely instability. Recent studies on the use of fall detection devices in the Malaysian community are scarce, despite the necessity to use them. Therefore, this study aimed to investigate the association between the prevalence of falls with instability. This study also presents a survey that explores older adults' perceptions and expectations toward fall detection devices. Methods: A cross-sectional survey was conducted involving 336 community-dwelling older adults aged 50 years and older; based on randomly selected participants. Data were analyzed using quantitative descriptive analysis. Chi-square test was conducted to investigate the associations between self-reported falls with instability, demographic and walking characteristics. Additionally, older adults' perceptions and expectations concerning the use of fall detection devices in their daily lives were explored. Results: The prevalence of falls was 28.9%, where one-quarter of older adults fell at least once in the past 6 months. Participants aged 70 years and older have a higher fall percentage than other groups. The prevalence of falls was significantly associated with instability, age, and walking characteristics. Around 70% of the participants reported having instability issues, of which over half of them fell at least once within 6 months. Almost 65% of the participants have a definite interest in using a fall detection device. Survey results revealed that the most expected features for a fall detection device include: user-friendly, followed by affordably priced, and accurate. Conclusions: The prevalence of falls in community-dwelling older adults is significantly associated with instability. Positive perceptions and informative expectations will be used to develop an enhanced fall detection incorporating balance monitoring system. Our findings demonstrate the need to extend the fall detection device features aiming for fall prevention intervention.

<p>〔目的〕軟性膝装具の装着圧力は，動作時に変化せず固定が不十分となるため，動作時に常に固定が得られるように，動的に装着圧力が変化する軟性膝装具が必要となる．本研究はそのような装具開発の基礎研究として，立ち上がり動作時の装着圧力の時間的変化を調べた．〔対象と方法〕対象は，健常若年男性18名とした．異なる装着圧力の軟性膝装具を使用した場合の立ち上がり動作時の装着圧力の時間的変化を調べた．〔結果〕立ち上がり動作の各相における装着圧力の変化において有意差が認められ，殿部離床時に装着圧力が最も大きくなった．また，膝関節屈曲角度に応じて装着圧力が高くなる傾向があった．〔結論〕立ち上がり動作時の軟性膝装具の装着圧力の変化および指標が確認できた．今後は，動的に装着圧力が変化する軟性膝装具の開発を進めていく．</p>

Monitoring of lumbar muscle activity during patient handling is important for the preventing lower back pain among caregivers. Electromyography is used for the measurement of muscle activity; however, EMG has some uncomfortable. Therefore, our previous study proposed measurement method for lumbar muscle activity using wearable sensors and found that the combination of wearable sensors and machine learning can be applied to estimate muscle activity. However, optimal machine learning-based predictor has not been explored. In addition, our previous method requires multiple sensors. Thus, this study explores optimal predictors to estimate lumbar muscle activity during patient handling by a single inertial sensor. The proposed method estimates lumbar muscle activity by acceleration and angular velocity on a trunk and machine learning-based predictor. In the evaluation, five machine learning-based predictors (artificial neural network, gaussian process, k-nearest neighbor, linear regression, and support vector machine) estimated lumbar muscle activity during patient handling. Mean absolute error values of lumbar muscle activity between each predictor and EMG were obtained as actual values. Results show that the proposed method using k-nearest neighbor could estimate muscle activity with minimal error. These results suggest that the proposed method using k-nearest neighbor as an optimal predictor can measure lumbar muscle activity.

Pointing devices commonly used today, including the modern computer mouse, can only be operated manually. For people with physical impairments, usage can be problematic due to a limited ability to operate such devices. Therefore, this study was inspired to conduct research on designing and evaluating an appropriate mouse-substitution system for individuals who are physically impaired. The design of the system uses an inertial measurement unit (IMU) that is a fusion of a gyroscope and an accelerometer sensor in which the sensor is attached to a user's upper arm to recognise physical gestures. Any gestures performed by the upper arm are then mapped and used to manipulate mouse cursor movements on computer devices. The design of the “clicking” method uses both an electromyograph (EMG) sensor and a bend sensor. This study evaluates two input devices; one is a combination of an IMU and an EMG sensor, and the other is an input device that is a combination of an IMU and a bend sensor. Fitts' Law formula and the ISO/TS 9241-411: Ergonomics of human-system interaction standard were used to evaluate quantitative performance and level of comfort. The quantitative results show that the average throughput (TP) of the first input device (2.30 bps) differs greatly, statistically, in comparison to the second input device (1.75 bps). Similarly, the average movement time (tm) revealed that there is a statistically significant difference between the first input device (1.98 s) and the second input device (2.67 s). The qualitative results show that the comfort levels of the first input device are superior to those of the second input device. It concludes that the combination of IMU and EMG as a pointing and clicking apparatus revealed better performance than the combination of IMU and bend sensor.
